OUR GOALS

At ACTIVE ADL, our primary objective is to value, empowerment and purpose for the time and energy of our members. The provider – client model have been meticulously developed using insights gathered from Health Care Partners actively involved in similar work.

Our vision centers around establishing a robust network of highly qualified Health Care Partners. To achieve this, every registered Health Care Partner members are actively employed and have at least 3 years of experience at a Skilled Nursing or Long-Term Care Facility. Every member is referred and works in a team . This ensures each Health Care Partner meets our stringent standards and this allows us select the most suitable professionals for our teams serving the community.

Within our structure, each Registered Provider operates within a bi-level management system.  Every Health Care Partner in the network has a supervisor who evaluates services and receives feedback for services rendered.

We recognize every member’s potential and envision them to lead and mentor others. We want our members to be empowered and to participate in growth.  As an ACTIVE ADL Health Care Partner, we encourage members to recognize their potential and have a mind set and drive to ask themselves how far can they take their potential and keep going forward.

Expectations from Health Partners

Our role extends beyond service provision; we are entrusted with upholding the caliber of service and adhering to established practice standards. Our two-tiered approach, with a supervisor serving as a team leader, serves multiple purposes. It guarantees a channel for receiving feedback and addressing any client or provider concerns. Furthermore, it fosters an environment of growth by cultivating stronger team and client relationships, using transactional feedback to continually enhance our service quality.
